Surviving a stroke is the easy part, however, for the survivor and their family, the reality of living with the devastating impact of a stroke when you get home, herein lies the challenge!
The Stroke Survivors Foundation, together with Strokefocus, launched PDSS (Post Discharge Stroke Support) for southern Africa, an innovative stroke care model that encourages survivors to sign-up before they leave hospital. The service is powered by a system developed by stroke support organizations around the world in a project named Strokefocus, operated by US based Wohaula LLC.
Currently, throughout the world, post-discharge stroke care falls short and leaves a lot to be desired. This leaves a major gap in post discharge care, which is particularly significant. PDSS aims to bridge the gap for survivors after they leave hospitals.
So why PDSS?
- Early contact means we have open channel of communication which allows for a long term relationship, making all the difference.
- Accessible to people who have suffered a stroke when they are still in early recovery mode and still fragile.
- No post discharge support = wasted acute care benefits
Post-stroke support can significantly uplift a survivors chances of a better recovery.
